	New Patients


	We welcome new patients. Our offices will accept both referrals from a referral center or directly. You can forward your medical records and insurance information to one of our offices. You should allow 30 minutes prior to arrival. Initial visits will take about 2-3 hours due to inital review of your history, medical records, and any recommended testing.


	Office Hours and Appointments

		Our office staff is available from 8:30 am to 5 pm, Monday thru Friday. We see patients by appointment only from 9 am to 4 pm, Monday thru Friday. Our office telephone number is (713) 797-1620. Cancellations should be made within 24hrs or as soon a possible.

	After Hours Telephone Calls

One of our physicians is always on call. Let our answering service know which physician you need to contact. State the nature and urgency of the call along with a current list of medications, and one of our physician will respond immediately.

If it is a true emergency do not hesitate to call 911 and ask to be transported to the St. Luke's Episcopal Hospital emergency room. One of the staff members will attend to your needs.

Our offices are closed the following holidays:

January 1 - New Years Day and January 2
Last Monday in May - Memorial Day
July 4 - Independence Day
First Monday in September - Labor Day
Last Thursday in November - Thanksgiving
Friday after Thanksgiving
December 25 Christmas Day and December 26


	Billing and Insurance

Fees for medical services performed in our office will be due and payable at the time of your visit. Our billing office will file all claims to third party contractors after you meet your co-pay and deductible. Should you have any questions concerning your insurance, please call our billing department at (713) 797-1620.

Bring all insurance cards and supplemental carrier information to your visit. We participate in Medicare, Medicaid and a host of HMOs and PPOs. Our staff will assist you with your questions. Please call our billing office to determine if we are participating providers for your insurance carrier.


	International Patients

		Our office staff is more than qualified to assist international patients. Call us at (713) 797-1620 we will be more than happy to answer any scheduling, billing or travel questions that might arise.
